BTW, I can definately see the Michael Close gaff in play on the video you provided...it's very plainly visible.
I disagree that there are only 2 methods to accomplish a moving hole effect. There are tons of ways to punch the first hole (It can start punched, suddenly apear punched, it can be punched by a spectator, punched by the magician, burnt ect.), tons of ways to seal up the first hole (you mentioned a flap but there are more, such as a switch, removing a piece of the card, removing a decoy hole ect.) and tons of ways for the hole to move across (With the help of a pen, visually, under the thumb, suddenly ect.)
