I use misdirection and get-readys. In the off-beat, just side jog the cards with your thumb and catch a pinkie break. It's quick to do the lift and natural looking.
Managing your audience is important, if somebody points out that you are lifting more than one card just speed along as if nobody said anything. Or play it off in another manner that you feel comfortable.
I'll give you an example. I was doing an effect where I DL and table the top card. The spectator asked if they could check the card to see if it was actually the card I had just showed them (which, obviously, it wasn't) I replied by saying 'In a moment'. When I revealed that the card wasn't the card that I had showed them, and that, that card was actually in my hands they were floored, they completely forget their suspicion. Just remember you're the magician, you make the rules and you control the flow.
It's bad if you're getting caught often. You will get caught from time to time. If you never get caught you're probably lying. And sometimes the spectator is just fishing and they get lucky.
I don't get caught often; I get caught once in a long while, but it's still frustrating.
I know what you mean about get- readies and misdirection.
I sometimes spread the cards out and say something like, "Your card could be any one of these," and close the deck with the top two cards together over a pinky break.
