In Street Magic, David Blaine is shown in a restaurant and he is sitting at a table with people around him. He takes out a new deck of cards. He has one of the spectactors open it and asked the spectator to shuffle the deck thoroughly. After that, he ask him to pick out any card in the deck and let everyone see while Blaine turns his head away. Blaine then instructs the spectator to put the card back in the deck and to shuffle thoroughly again. He asked the spectator to then spread out the cards all over the table. Blaine then covered the cards with a menu and he takes a steak knife and jabs the knife into the menu. He leans the table over to let the cards fall and the knife is found to be stuck through the card that was selected. Can anyone tell me what is this trick called?

It's called the "Card Stab". There are several versions of it. Do a search on google.com and you'll find a bunch of info on it.

Is this trick gaffed, or pure slight of hand?

See, I came up with my own version, but when I recently watched DB show, David gave an unopened pack to the spec, told him to open the pack and take any card he wanted. David didn't even touch the pack. The only way I can see this working is if it's edited out or by card control and a bit of palming, as well as misdirection.
